Duties:

| 1 | Obtains and records patient medical history and personal data. |
| 2 | Assesses patients on initial visit and each follow up visit. |
| 3 | Takes and records vital signs, blood pressure, temperature and respiration on each visit. |
| 4 | Assists physician with patient examinations. |
| 5 | Explains physician instructions if needed. |
| 6 | Prepares and administer medications as directed by the physician. |
| 7 | Provides service in accordance with reimbursement, ethical and legal guidelines. |
| 8 | Informs the physician and other related personnel of changes in the patient’s needs and/or physical changes. |
| 9 | Monitors refrigerator temperatures for vaccines. |
| 10 | Assures all medications and sharps are properly stored and locked at all times as per AHCA regulations. |
| 11 | Monitors medication expiration dates. |
| 12 | Works closely with physicians to assure patient’s HEDIS measures are being met. |
| 13 | Writes daily notes according to the Federal and State guidelines. |
| 14 | Communicates with the physician regarding patient’s status, obtaining additional orders if necessary. |
| 15 | Observes confidentiality and safeguards on all patient-related information and medical facility operational data. |
| 16 | Completes order status and immunization records. |
| 17 | Performs phlebotomist duties such as, collecting and preparing laboratory specimens. |
| 18 | Ensures that all HIPAA/OSHA standards are in compliance. |
| 19 | Catalogs and communicates patient needs to the PCP, Clinician, Specialist and/or other clinical personnel. |
| 20 | Prepares patients for examinations and performs routine screening tests; assists physician with exams; explains treatment procedures and physicians’ instructions to patient. |
| 21 | Observes patients and reports any changes in patient conditions to the PCP, Clinician, Specialist and/or other clinical personnel. |
| 22 | Perform non-critical medical and therapeutic procedures based on medical instructions. Collects phlebotomy and other lab specimens; performs basic waived lab tests. |
| 23 | Prepares and administers medications; changes dressings, applies bandages, removes sutures and other first aid procedures; uses CPR skills when necessary, all under physician supervision. |
| 24 | Maintains supplies, equipment, stocks and sterilizes instruments; practices OSHA safety standards; performs accurate, legal, and ethical documentation at all times. |
| 25 | Processes patient phone messages, returning calls and routing them to other team members as appropriate; calls patients to obtain and relay pertinent information for the physician. |
| 26 | Upon physician approval and authorization, calls in prescriptions to the pharmacy. Uses communication skills with appropriate medical terminology; and follows appropriate legal and ethical professional conduct; authorized individuals will use the ChenMed Rx system to support physician medication dispensing. |
| 27 | Provides health coaching to a defined group of patients to support healthy lifestyle choices. Follows up with coached patients via weekly calls. |
